The following are forms and information courtesy of various groups.

1. Planned Parenthood of Middle Tennessee PG-13 Players Contract Agreement.

PLANNED PARENTHOOD OF MIDDLE TENNESSEE PG-13 PLAYERS CONTRACT AGREEMENT

I. PROFESSIONAL CONDUCT POLICIES
     A. Training, Meetings, Rehearsals: Player will arrange for
player's own transportation to and from PG-13 Players
activities (training sessions, meetings, rehearsals,
performances (to and from D.B. Todd Clinic), and cast
parties). Player will attend all training sessions in August,
one Full Cast meeting per month, and team rehearsals as called
by Coordinator/Drama Consultant. In the event of emergency
absence, Player will notify the Coordinator by telephone at
beeper number. Absence from rehearsal without notification
constitutes a grievance between Coordinator and Player.
Player's absence from rehearsal may result in Player being
replaced for the performance in question.

     B. Promptness: Player will arrive at announces starting
time of all training sessions, cast meetings, and team
rehearsals. Upon arrival, Player will note and sign time of
arrival in Attendance Log. In the event of emergency, Player
will notify Coordinator by telephone, at beeper number, prior
to late arrival if possible. Upon arriving late for meeting
or rehearsal, Player will note and sign time of arrival in
Attendance Log, and unobtrusively join in activities. At
break time or at the end of the rehearsal, Player will
approach Coordinator/Drama Consultant to discuss lateness.
Unreasonable or too-frequent lateness, or lateness that
inhibits the progress of rehearsals, constitutes a grievance
between Coordinator/Drama Consultant and Player.

     C. Preparation for Rehearsals: Player will meet announced
deadlines for memorization of all assigned roles. Throughout
the season, Player will maintain these roles, and any
additional roles assigned by Coordinator/Drama Consultant.
Failure to commit lines to memory that inhibits the progress
of rehearsals constitutes a grievance between
Coordinator/Drama Consultant and Player.

     D. Full Cast Meetings, Scheduling: Full Cast Meetings will
take place during the first week of each month. At each Full
Cast Meeting, pLayer will be required to complete a Volunteer
Time Sheet showing the number of hours spent per month in PG
13 Player activities (meetings, rehearsals, performances). In
case of absence from Full Cast Meeting, Player must make
arrangements to complete this Time Sheet within one week. To
each Full Cast Meeting, Player will bring personal calendar
with complete record of academic, extracurricular, religious,
and family commitments for the next five weeks. At full Cast
Meeting, Player will commit to rehearsal dates, performance
dates and roles for the next five weeks by signing
Coordinator's sheet. Once Player has committed to a
performance, Player may break the commitment only in case of
reasonable conflict or emergency. "Reasonable conflict" is
defined as unchangeable family or academic commitment or
other conflict determined reasonable by Coordinator.
Unreasonable or too-frequent cancellations on the part of the
Player constitute a grievance between Coordinator and Player.
In case of later discovery of reasonable conflicts with
performance, Player will notify Coordinator at Planned
Parenthood office number as soon as possible after learning
of the conflict. If Player is leaving a message, Player will
use the following format: "This is (name). I will not [or may
not] be able to perform the roles of (character[s]) at
(location) on (date) because of (conflict). You can reach me
at (number)."

     E. Performances: Coordinator will provide written
performance information (skits to be performed, time and
place to meet before performance, performance location
contact number, and expected time of return to PP offices) to
Player no later than five days before performance. If Player
wishes to meet Coordinator at performance location rather
than at D. B. Todd Clinic, player must provide confirmed
notice to coordinator: Player must either speak directly to
Coordinator on the telephone, or if leaving a message,
request confirmation call from Coordinator. If Player does
not receive confirmation, Player must meet at D. B. Todd
Clinic at call tine. Unreasonable or too-frequent lateness
for performances constitutes a grievance between Coordinator
and Player.Once Player has committee to a performance,
missing that performance without notification may
constitutes grounds for immediate dismissal. In case of
emergency absence, Player will notify Coordinator by
telephone at beeper number. If Player is unable to contact
Coordinator before performance, Player will continue to make
all possible efforts to reach Coordinator to discuss absence.
Failure to notify Coordinator in case of absence constitutes a
grievance between Coordinator and Player. 

     F. Performance Uniform: Player will wear official PG-13
Players uniform (clean logo t-shirt and clean black
"bottoms:" pants, skirt, leggings, or shorts) in all
performances or public appearances as a PG-13 Player. Failure
to wear uniform constitutes a grievance between Coordinator
and Player. Player will receive one t-shirt at beginning of
season. If t-shirt becomes unwearable (torn, stained, etc.)
or is lost, Player will purchase replacement t-shirt at a
cost of $8.00.

II. GRIEVANCE POLICIES

Guiding assumptions for the PG-13 Players program are that
all participants will treat each other with dignity,
respect, and professionalism. With these basic assumptions
in mind, the following policies have been designed to
facilitate and record the resolution of conflicts between
participants. In applying these Grievance Policies, the
professional standards of dignity and respect for others
shall be assumed.

     A. Between Coordinator/Drama Consultant and Player: The
following procedure will be used for handling grievances
between Coordinator/Drama Consultant and Player:
          1. Player will be given verbal warning of the
grievance, in a private manner, by the Coordinator/Drama
Consultant. Coordinator/Drama Consultant will note the date,
time, and reason for this warning on a Grievance Sheet, to be
placed in the Grievance Policy Log. This log is confidential,
seen and maintained only the Coordinator.
          2. If the problem persists, Player will be given
written notice of Probationary Status. Player will outline
and sing written plan to address the problem. Duration of
Probationary Status will be determined by the
Coordinator/Drama Consultant, depending upon the severity of
the grievance. As much as possible, each grievance will be
handles separately, with its own term of probation. At the end
of this term of Probationary Status, the grievance is
considered canceled.
          3. If the problem recurs during the duration of
Probationary Status, or if Players incurs another grievance,
Player will be dismissed from cast.

     B.Between Player and Coordinator/Drama Consultant: The
following procedure will be used for handling grievances
between Player and Coordinator/Drama Consultant:
          1. Player will directly approach the person with whom
Player has grievance, in a private manner, to discuss
problem. Player will note the date, time, and reason for this
conversation on a Grievance Sheet to be placed in the
Grievance Policy Log.
          2. If problem persists, Player will submit written
notice of grievance to coordinator/Drama Consultant, a copy
of which shall be placed in the log. Coordinator and Drama
Consultant will meet to outline plan to address problem.
          3. If problem is still not resolved, Player will
request meeting with director of Education to discuss
problem.

     C. Between Player and Player: The following procedure will
be used to handle grievances between one Player and another:
          1. Player will directly approach the person with whom
Player has grievance, in a private manner, discuss the
problem.
          2. If problem is not resolved, Player will request a
meeting with other Player and Coordinator and/or Drama
Consultant to discuss problem.
          3. All parties will outline and sign a plan for
addressing the problem.
          4. Player's failure to adhere to this plan
constitutes a Grievance between Coordinator/Drama Consultant
and Player.

     D. Grounds for Immediate Dismissal: The following
grievances may be grounds for immediate dismissal from the
cast, without application of the above Policies:
          1. Player's arrival at any PG-13 Players activity
(rehearsal, meeting, training session, performance, or cast
party) under the influence of drugs or alcohol.
          2. Player's illegal actions while involved in any PG
13 Players activity.
          3. Player's violence towards any other person while
involved in any PG-13 Players activity.
          4. Player's unwanted sexual advances, sexually
harassing comments, or inappropriate touch towards any other
person while involved in any PG-13 Players activity.
          5. Player's absence from a performance to which
Player has agreed, without notification before or after the
missed performance.

IV. ATTACHMENTS
     In addition to the policies and procedures outlines in the
contract above, as a Part-Time Contract Employee of Planned
Parenthood of Middle Tennessee, Player will adhere to
applicable agency policies and procedures, including
documents attached: Confidential Agreement and Teen Theatre
Cast Member Job Description.

I have read and had explained to me the policies and
procedures in the Contract Agreement. By signing below, I
agree to adhere to this Agreement (including the attached
documents) during my tenure as a PG-13 Player.
